Support Worker Training in High-Intensity Skills

We provide comprehensive, participant-specific training to equip support workers with the clinical knowledge, practical skills, and confidence required to safely support individuals with complex care needs. All training is aligned with NDIS High Intensity Daily Personal Activities (HIDPA) guidelines.

Training is tailored to each participant’s care plan and may include:

Complex bowel care

Urinary catheter care (indwelling, intermittent, and suprapubic catheters)

Enteral feeding support (PEG/PEJ)

Subcutaneous injection support

Pressure injury prevention and wound care management

Medication management support

Our goal is to ensure support workers are confident, competent, and consistent in delivering safe, person-centred care that meets individual needs.

 

Clinical Nursing Oversight

We provide specialised nursing support for participants with complex clinical needs requiring a higher level of care, clinical nursing oversight, and coordination.

 

We work collaboratively with participants, families, support workers, and allied health professionals to ensure care remains safe, effective, and aligned with individual care plans and clinical best practice.

 

Post-Hospital Care

We provide short-term nursing support following surgery, illness, or hospital discharge to promote a safe and supported recovery at home or in the community.

Our team assists with wound care, medication management, and ongoing clinical monitoring to ensure recovery is progressing well and any complications are identified early.

 

Medical Appointment Support

We provide support for participants to attend medical appointments, ensuring they feel comfortable, informed, and supported throughout the process.

We can assist with:
• Accompanying participants to medical and specialist appointments
• Providing clinical support and advocacy during consultations
• Communicating relevant health information with healthcare providers
• Assisting with follow-up care and treatment plans

Our aim is to ensure clear communication between all parties and support participants to feel confident and well-supported throughout their healthcare journey.

 

Multidisciplinary Team (MDT) Meetings

 

Complex health needs require a team-based approach. MDT meetings bring together relevant therapy and healthcare professionals to ensure coordinated, holistic, and person-centred care planning. We support clear communication and collaboration between GPs, specialists, allied health professionals, and support providers to ensure care is consistent and aligned with individual needs and goals.

 

This holistic approach ensures no detail is overlooked, supporting safer, more efficient, and person-centred outcomes.
