A Nurse-Led Service You Can Trust
Perth Disability Care is a small, nurse-led disability and community nursing service in Perth, delivering specialist continence care, community nursing, and complex clinical care for adults and children.
Our care is guided by a simple principle: every person deserves safe, respectful, and evidence-based nursing care delivered with dignity in their own home.
We work closely with participants, families, carers, and support teams to deliver coordinated, person-centred nursing care across Perth. Our services include clinical nursing, continence care support, education, and ongoing clinical guidance that strengthens confidence, promotes independence, improves health outcomes and quality of life.
Through clinical expertise and genuine compassion, we provide care that is tailored, consistent, and meaningful—helping people live safely, independently, and with dignity at home.
Our Approach to Care
We understand that living with disability, chronic illness, neurological conditions, or age-related health needs can be complex.
That’s why we focus on:
- Preventative care and early intervention
- Evidence-based clinical nursing practice
- Education for families and support workers
- Consistent, in-home nursing support
- Improved long-term health outcomes
Our goal is to reduce hospital admissions, improve daily functioning, and support people to live safely and independently at home.
At Perth Disability Care, we combine clinical expertise with a respectful, person-centred approach—ensuring every individual feels supported, valued, and understood.
Perth Disability Care
Meet our nurse Anu
With over 15 years of experience in hospital and community nursing in Perth, I have supported people living with complex health conditions, including continence and disability-related care needs.
I take a person-centred approach, delivering practical, evidence-based nursing and disability support tailored to each person’s home, routine, and goals. I work closely with participants, families, and support teams to provide realistic strategies that improve comfort, independence, and overall wellbeing.
Community nursing requires flexibility, clinical judgement, and consistency. I focus on delivering safe, reliable care that supports independence and continuity in the home.
For me, nursing is about more than treatment—it is about providing compassionate, consistent care that helps people feel supported, understood, and confident in managing their health at home.
What matters most to me is ensuring people feel less alone in their care journey and supported to build confidence, comfort, and independence.
My goal is to walk alongside people throughout their care journey, providing the right guidance to improve wellbeing and quality of life.
