What is a Continence Assessment?

A continence assessment is a comprehensive clinical evaluation completed by a Continence Nurse Specialist to identify bladder and bowel concerns and recommend the most appropriate management strategies and supports including consumables prescription and quote.

Our assessments can assist with:

• Urinary and faecal incontinence

What Happens During the Assessment?

During the assessment, our Continence Nurse Specialist will assess participant’s health and continence history, daily routine, and current continence concerns to understand the factors affecting their bladder or bowel function.

We review medical history, medications, diet and fluid intake, mobility and toileting access, and frequency and severity of symptoms.

Preparing for the Assessment.

To ensure a thorough assessment, please have available a list of current medications, relevant medical history, recent health summaries or hospital discharge reports, and details of any healthcare professionals involved in your care.

This information helps our Continence Nurse Specialist to gain a clear understanding of your needs and provide tailored recommendations and management strategies.

What Happens After the Assessment?

Following the assessment, our Continence Nurse Specialist prepares a comprehensive continence report outlining assessment findings, recommended management strategies, and individualised continence care recommendations. The report also includes advice on suitable continence consumables, equipment requirements, and a detailed cost estimate to support funding requests. This provides participants, families, support coordinators, and NDIS planners with a clear understanding of the participant’s continence needs and the anticipated costs involved in managing those needs effectively.

Continence Assessments for Children

We support children experiencing continence challenges by working closely with families to develop age-appropriate management plans tailored to their individual needs. Where required, we can also provide guidance and documentation for schools, teachers, and support workers to help ensure consistent support across different environments.

Telehealth Consultations

Perth Disability Care offers telehealth continence consultations, providing convenient access to clinical support from home. This service is suitable for participants living in rural or regional areas, or individuals unable to attend a face-to-face appointment.

All video and telephone consultations are delivered through a secure, privacy-protected platform, providing timely access to specialist clinical advice and recommendations without the need for travel.
